Work Hours/Shift:

  • Full-Time, Monday – Friday

You Will Be Responsible For:

  • Direct and oversee education, orientation, competencies and specialty training for all Home-Based Care Service Lines.

  • Demonstrate the ability to influence work outcomes through others while maximizing opportunities for standardization and integration of ‘best practices’ within the organization.

  • Ensure competency of all Home-Based Care staff: clinical and non-clinical. Ensure documentation of competency meets all regulatory requirements.

  • Leads teams in the design and deployment of fiscally responsible, patient-centered processes of care.

  • Leads development of orientation, onboarding and new hire retention programs.

  • Develops, mentors, and coaches leadership to confirm and increase leadership competency.

  • Acts as liaison to Information Systems: functions as a content expert, provides quality assurance, leads system development.

Qualifications

What You Will Need:

  • Bachelor of Science in Nursing degree from an accredited school of professional nursing

  • Background in quality improvement, clinical regulatory, clinical education and clinical quality programs

  • Current Registered Nursing License

  • Five years home health/hospice experience in the last ten years required

  • Excellent understanding of Homecare Regulations, Medicare Conditions of Participation, OASIS

  • Experience with all types of clinical training, including non-nursing positions, possesses a clinical perspective with other roles such as therapists and medical social workers.

  • Experience working with adult learners.

  • Two years’ experience in quality and/or patient safety activities including regulatory compliance, survey process, certification and enforcement of regulations

  • Understands home health and hospice public reporting requirements, STAR Ratings, VBP and related process improvement methodologies

  • OASIS Certification preferred

Job Summary:

The Director will have primary responsibility for the education and quality activities of all home-based services programs of Advent Health. Through supervision and collaboration, the director will lead development of field, office and administrative staff orientation, competency and ongoing educational programs, and the development of patient education and patient facing material, to maximize efficiencies, promote quality care and promote standardization across the organization.

In addition, the Director will have primary oversight of the quality and patient safety program for Home Based Services. This leadership role will support others within home-based services in robust clinical process improvement supporting evidence-based practices, the reduction of medical/health care errors and other factors that contribute to unintended adverse outcomes and improved compliance with regulatory and accreditation standards.
