The role you’ll contribute:

The Clinic Lead provides leadership and team management for the nursing staff, medical assistants and provides input to the management of patient service coordinators, and laboratory/radiology technicians in the Medical Center. The Clinic supervisor administers clinical policies to ensure the provision of quality patient care, efficient use of resources, compliance with regulatory policies and patient satisfaction with services at Gordon Hospital’s Physician Practice

  1. The nurse is responsible for execution of the Nursing Process:

1.1 Assessment – The nurse collects patient health data.

a. Data collection involves the patient, family, and other health care providers as appropriate.

b. The priority of data collection activities is determined by the patient’s immediate condition or needs.

c. Pertinent data are collected using appropriate assessment techniques and instruments.

d. Relevant data are documented in a retrievable form.

e. The data collection process is systematic and ongoing.

1.2 Diagnosis – The nurse analyzes the assessment data in determining diagnoses.

a. Diagnoses are derived from the assessment data.

b. Diagnoses are validated with the patient, family, and other health care providers, when possible and appropriate.

c. Diagnoses are documented in a manner that facilitates the determination of expected outcomes and plan of care.

1.3 Outcome Identification – The nurse identifies expected outcomes individualized to the patient.

a. Outcomes are derived from the diagnoses.

b. Outcomes are mutually formulated with the patient, family, and other health care providers, when possible and appropriate.

c. Outcomes are culturally appropriate and realistic in relation to the patient’s present and potential capabilities.

d. Outcomes are attainable in relation to resources available to the patient.

e. Outcomes include a time estimate for attainment.

f. Outcomes provide direction for continuity of care.

g. Outcomes are documented as measurable goals.

1.4 Planning – The nurse develops a plan of care that prescribes interventions to attain expected outcomes.

a. The plan is individualized to the patient (e.g., age-appropriate, culturally sensitive) and the patient’s condition or needs.

b. The plan is developed with the patient, family, and other health care providers, as

appropriate.

c. The plan reflects current nursing practice.

d. The plan provides for continuity of care.

e. Priorities for care are established.

f. The plan is documented.

1.5 Implementation – The nurse implements the interventions identified in the plan of care.

a. Interventions are consistent with the established plan of care.

b. Interventions are implemented in a safe, timely, and appropriate manner.

c. Interventions are documented.

1.6 Evaluations – The nurse evaluates the patient’s progress toward attainment of outcomes.

a. Evaluation is systematic, ongoing, and criterion-based.

b. The patient, family, and other health care providers are involved in the evaluation process, as appropriate.

c. Ongoing assessment data are used to revise diagnoses, outcomes, and the plan of care, as needed.

d. Revisions in diagnoses, outcomes, and the plan of care are documented.

e. The effectiveness of interventions is evaluated in relation to outcomes.

f. The patient’s responses to interventions are documented.

  1. The RN or LPN practices in accordance to the established Georgia Nurse Practice Act and law.

  2. The RN or LPN is responsible for maintaining general mandatories established by Gordon Hospital and any unit-specific mandatories, as deemed necessary for practice within that setting.

  3. The RN or LPN is responsible for legal, ethical practice. The nurse serves as a patient advocate in accordance with the patient bill of rights. The nurse is responsible for compliance with regulatory standards pertinent to nursing practice.

  4. Assists in coordinating and providing quality patient-focused care in a physician’s clinic.

  5. Supervise and support clinical staff through the disciplinary and performance appraisal evaluation and management of clinical office staff.

  6. Perform patient assessment, vital signs, lab tests, procedures, and process x-rays per physician’s orders.

  7. Prepare exam room for next patient.

  8. Assist physician(s) with procedures as necessary.

  9. Communicate with patients regarding labs, meds and referrals.

  10. Maintain supplies and inventory.

  11. Collaborates with laboratory director and clinics to establish appropriate policies and Procedures.

Qualifications

The expertise and experiences you’ll need to succeed :

· High School plus some College

· Current Georgia LPN license

· > 3 years medical experience

· Phlebotomy experience

· X-ray experience

· > 1-year LPN experience
