The community you’ll be caring for: Adventhealth Daytona Beach

The role you’ll contribute: Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization. Provides Patient/family instruction, scheduled physician appointments, any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.

•Follows established criteria for utilization of services to ensure denials are avoided

•Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities

•Acts as liaison with physician to assure appropriate and complete documentation •Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision

•Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning

•Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities

•Reviews patient medical records for PI / Risk Management per established criteria and provides appropriate documentation of adverse patient occurrences

Qualifications

The expertise and experiences you’ll need to succeed :

Minimum qualifications :

High School Grad or Equivalent

Three years LPN health care experience

Preferred qualifications:

· 5 years' clinical experience LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: • Licensed Practical Nurse /Vocational Nurse
