The role you’ll contribute:

Participates in patient assessment, counseling, support, and resource linkage. Provides counseling to patients and families as well as organizing and leading support groups. Works closely with physicians and other medical staff to aid patients in understanding treatment options, facilitating decision making, as well as providing a variety of other case work services in acute care areas, outpatient, and in the community.

  • Assesses patient’s psychosocial history, cultural and spiritual needs through observation, interviews, and clinical data.

  • Identifies patient needs and current resources (e.g., financial, patient rights, insurance, family support).

  • Provides social casework services as part of an interdisciplinary approach.

  • Participates in rounds, case conferences and committees to facilitate appropriate patient services.

  • Provides support/interpretive services to physicians and hospital staff regarding patient’s psychosocial needs in order to enhance medical care.

  • Given an understanding of the impact of social/agency policy, choosing interventions to enhance patient’s ability to function successfully in their environments.

  • Functions as a patient advocate by communicating with patient, family, and staff regarding ethical dilemmas and identifying patient needs unmet by current resources and exploring alternate methods to meet patient needs.

  • Given knowledge of relevant federal and state laws, coordinating necessary legal interventions in provision of care and discharge planning (i.e., guardianship and conservatorships, POA, etc.).

  • Identifies impact of disease processes and human psychology on patients and families.

  • Recognizes ethnic, cultural, and disabling human diversity factors in order to develop trusting work relationships with patients, families, and staff.

  • Demonstrates the ability to build a trusting relationship with patients and families that will aid in adjustment to illness, loss, and decision making regarding the patients’ treatment and care.

  • Mediates conflicts among patients, families, and staff.

  • Provides education to increase patient and family knowledge of illness, health maintenance/treatment options, and use of appropriate level of care.

  • Organizes, promotes and leads support groups.

  • Provides concise verbal communication and written documentation to patients/families and staff regarding assessment and other pertinent issues.

  • Ability to prioritize and balance the various and at times irreconcilable interests of the patient, the insurance companies, medical staff, and the available programs and facilities.

  • Consistently complies with all requirements of Federal and State laws, all regulatory agencies and accreditation standards, ethics guiding the behavioral health profession and Centura policies and standards of care.

  • Ability to provide on-call coverage outside of standard scheduled hours per department and/or facility policy.

  • Clinical documentation, including treatment plans, ongoing status, interventions, patient response and outcomes in an accurate and timely manager in accordance with managed care provider and Centura standards.

  • Provides timely, clear written and verbal communication to relevant providers, managed care, and patient’s family members as appropriately.

  • Maintains confidentiality regarding patient assessment, interventions and status.

  • Enhances professional growth and development through educational programs, current literature, in-service meetings and workshops to remain current with industry changes.

  • Participates and supports other department activities and community services. Develops, organizes and conducts education sessions for patients, family members, physicians and staff.

  • Recognizes opportunities for performance improvements and initiates efforts for system improvements.

Qualifications

The expertise and experiences you’ll need to succeed :

Minimum qualifications :

  • Master’s Degree in Social Work, Counseling or related

  • One year of related clinical experience

  • Demonstrates clinical and ethical professionalism, conflict resolution and critical thinking skills.

  • Current State License as a Clinical Social Worker (CSW), Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)

  • Current Basic Life Support from the American Heart Association
