The role you'll contribute:

Assesses, plans, implements, and evaluates all aspects of patient’s nutrition care throughout all phases of transplant. During the pre-transplant phase, the dietitian provides an in-depth patient nutritional assessment, develops an appropriate plan of care to maintain/improve the patient’s nutritional health, is responsible for the nutrition-related education of the patient and provides recommendations to the Patient Review Committee during the transplant selection process. During the transplant phase, the dietitian coordinates with the inpatient dietitians and the multi-disciplinary team as necessary or clinically indicated. In the post-transplant phase, the dietician is involved with routine nutrition support & patient education as clinically indicated. Occasional travel may be required to perform patient care with multidisciplinary team on outreach locations.

  • Researches, develops, and revises education material as needed, ensuring evidenced-based guidelines are being followed.

  • Obtains medical and non-medical information through interviewing the patient and/or caregivers and reviewing medical record.

  • Utilizes hospital policies and protocols to provide timely and appropriate nutrition intervention and therapy that is age-specific, culturally specific and maintains/improves the nutritional status of the patient if possible.

  • Provides nutrition education to patient and other appropriate individuals regarding nutrition-related concerns that may increase patient’s risk for transplant surgery and recommends nutrition interventions to reduce these risks.

  • Attends and participates in transplant team meetings including patient review committee, hospital interdisciplinary rounds, discharge planning, and QAPI with the ability to communicate with the transplant team in a professional and effective manner.

  • Assists with the on-boarding, training and professional development of Transplant Specialty Dietitian and other members of the clinical team.

  • Shares information with nutrition staff members and medical team. Informs health team members about codes, regulations, professional guidelines, departmental policies, and procedures that are related to nutrition care as needed.

Qualifications

The expertise and experiences you’ll need to succeed:

Required:

  • High School Diploma or GED

  • Bachelor’s degree in food & nutrition, Dietetics, Nutritional Sciences, Human Nutrition or related field and successful completion of a CADE or ACEND accredited dietetic internship program.

  • Current Registered Dietitian

  • More than two years clinical experience as a Registered Dietitian in a complex healthcare setting.

Preferred:

  • Master’s level degree in Food & Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or related field preferred

  • Previous transplant or dialysis experience
