The role you’ll contribute (Job Summary):
Has knowledge of community health concepts, principles of ambulatory clinical nursing care, case management and care coordination. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient self-management efforts. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrate personal accountability for relationship-based care, organizational mission and core values.
The value you’ll bring to the team (Role/Responsibilities):
· Assess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. Evaluates patient’s response and intervenes to ensure optimal patient outcome.*
· Assess patient’s ability to participate to the fullest extent by determining presence/ absence of barriers to active participation in the individualized care plan.*
· Develop and implement patient plan of care and observe outcomes in accordance with nursing standards and in collaboration with the interdisciplinary team.*
· Demonstrate competency in a variety of therapeutic/diagnostic
· interventions including but not limited to IV’s, incision/wound care, medication administration, catheterization and specimen collections. *
Qualifications

Required:
· Education:
o Degree: Associate
§ Specialty: Nursing
· Comment: Graduate of an accredited School of Nursing
· Credential: RN
o Comment: Current state RN license/ Current RN license from a participating state in National Licensure Compact
· Credential: BLS
o Source: American Heart Association
§ Current: BLS
Preferred:
· Education:
o Degree: Bachelor
§ Specialty: Nursing
· Comment: BSN Preferred, encouraged to obtain within 3-years of assuming role.
· Experience:
o One year experience preferred. Nursing skills and knowledge base specific to specialty.
