What You Will Be Responsible For:
1. Leads Infection Prevention Efforts Across Physician Enterprise
1.1 Serves as an expert regarding CDC guidelines for infection prevention in healthcare and definitions of healthcare associated infections, TJC standards, HFAP standards, and federal and state regulations.
1.2 Serves as an expert regarding infection prevention processes in the physician office setting.
1.3 Attends continuing education programs and maintains current knowledge of aspects of infection prevention.
1.4 Maintains knowledge of microbiology and modes of transmission of disease entities.
1.5 Teachesprinciples and practical applications of infection prevention to all levels of healthcare personnel.
1.6 Develops and implements a system for identifying, investigating, reporting, and preventing the spread of healthcare associated infections among patients of all ages and healthcare personnel relative to physician office practice settings.
1.7 Confers with staff within the Physician Enterprise practices on a regular basis to determine occurrence of practice (healthcare) associated infections.
1.8 Investigates infections and/or in clusters by physician practice.
1.9 Reviews aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
1.10 Collaborates with Associate Health and assists with follow-up of accidental exposure incidents, as indicated.
1.11 Completes required communicable disease reports to the public health department in a timely and accurate manner.
1.12 Maintains accurate records of physician practice (healthcare) associated infections among patients and employees.
1.13 Assists in the development, implementation, and improvement of infection prevention practices within Physician Enterprise. Ensure that practices are consistent with those of the hospital.
1.14 Monitors proper use of germicides, cleaning products, antiseptics and disinfectants in use throughout Physician Enterprise. Ensure that processes are consistent with those of the hospital.
1.15 Monitors methods of asepsis, sterilization, and disinfection employed throughout Physician Enterprise. Ensure practices are consistent with those of the hospital.
1.16 Reviews and/or revises Physician Enterprise policies and procedures related to infection prevention. Analyzes policies for internal consistency and compliance with accreditation standards through the evaluation of information.
1.17 Communicates infection prevention activities to administration, Environment of Care (EOC) Committee, the medical staff and Medical Executive committee via Quality Committee minutes and/or the Quality/Infection Prevention Committee Chairperson and hospital Infection Preventionist.
1.18 Collaborates with the Quality/Infection Prevention Committee facilitator and hospital Infection Preventionist to review and approve Physician Enterprise Infection Prevention policies and procedures at least every years.
1.19 Based on studies which identify areas for improved patient outcomes, submits targeted surveillance indicators for Quality/Infection PreventionCommittee approval on an annual basis.
1.21 Establish effective interpersonal relationships with physicians, nurses, and members of the healthcare team.
1.22 Designs and implements new methods to improve the efficiency and outcomes of the Infection Prevention Program within the Physician Enterprise settings.
1.23 Serve as an expert resource on accreditation and regulatory issues as it relates to Infection Prevention for Physician Enterprise.
1.24 Provide consultation to relevant task forces and committees to insure ongoing compliance with Infection Prevention standards.
1.25 Facilitates physician office compliance with infection prevention standards
2. Facilitates Regulatory/Accreditation Compliance Efforts for Physician Enterprise
2.1 Maintains knowledge of national, state and local regulatory and accreditation requirements, as it pertains to the North Carolina State Statutes, CMS Conditions of Participation (CoP), Joint Commission (TJC) standards, and Healthcare Facility Accreditation Program standards, as it relates to the operations of Physician Enterprise.
2.2 Provides direction to ensure that clinical services are provided in accordance with safety standards established by these agencies.
2.3 Responsible for facilitating improvement efforts to ensure compliance with TJC and HFAP Accreditation and CMS Conditions of Participation by ensuring continuing survey readiness.
2.4 Conducts mock surveys and tracer activities across the Physician Enterprise on a regular basis.
2.5 Collaborates with the Patient Safety Specialist to collect data (chart audits) to assess for compliance with National Patient Safety Goals.
2.6 Prepares results and/or summaries of data collection, including graphic presentation.
2.7 Assists the Clinical Risk/Accreditation Manager to complete the Focused Standard Assessment for accredited programs
2.8 Communicates with and educates associates and medical staff on TJC and HFAP standards
2.9 Acts as the liaison for surveyors during on-site survey activities (preparation, planning, coordinating, etc.)
2.10 Communicates and facilitates necessary changes in order to remain continuously compliant with accreditation standards.
2.11 Proactively seeks opportunities/mechanisms aimed at continually improving patient care, processes and compliance.
2.12 Facilitates the creation of an effective action plan in response to identified issues.
What You Will Need:
Graduate of an accredited school of nursing required
Current RN license in the state of NC required
Minimum 5 years nursing care experience required
Current North Carolina driver's license in good standing required
Current auto insurance policy according to minimum NC stat requirements, required.
Obtains certification in Infection Control & Epidemiology (CIC) within 2 years of start of employment required
Obtains membership in the Association of Practitioners of Infection Control and Epidemiology (APIC) after start of employment, required.
Working knowledge of microbiology, epidemiology, infectious diseases, aseptic techniques, and current practices relative to infection prevention required.
Able to communicate effectively in English, both verbally and in writing required.
Master's degree preferred
Experience in infection prevention preferred and healthcare accreditation preferred
Experience working in a physician office preferred
Knowledge of CDC guidelines and definitions of healthcare associated infections, TJC standards, HFAP standards, and federal and state regulations
Knowledgeable of CDC guidelines and definitions of healthcare associated infections, the Joint Commission (TJC) and Healthcare Facilities Accreditation Program standards, and federal and state regulations. Responsible for the identification, investigation, reporting, prevention and control of healthcare associated infections among patients and personnel in the Physician Enterprise offices. Responsible for assisting with in-service programs related to infection prevention and accreditation requirements. Works collaboratively with the Infection Preventionist, the Clinical Risk/Accreditation Manager, and Physician Enterprise leaders to ensure continuous readiness and compliance with all accrediting body and regulatory agency requirements, across the Physician Enterprise.
