PRINCIPAL DUTIES AND JOB RESPONSIBILITIES :

  • Responsible for the review of patient care using established criteria as determined by Federal and State health care regulations, accrediting bodies, the Quality Manager, and organizational leadership.

  • Accurately completes patient data abstraction daily and identifies potential quality of care issues to the appropriate departmental leaders and medical staff.

  • Accurately completes patient data abstraction and analyses for quality of care issues as requested by Quality Manager for on-going process review.

  • Prepares and presents reports on data collected for departmental directors, Quality Committee and other internal sources as requested.

  • Assist in facilitating use of data to develop proactive measures to improve clinical outcomes, patient safety and hospital processes by collaborating with Quality Manager, individual department leaders and other organizational leaders as deemed appropriate.

  • Provides timely and accurate education to direct care staff and physicians on new federal and state quality measures as well on noted quality issues.

  • Track noted issues for improvement and monitor for trends over time. Provide Quality Manager and appropriate departmental leaders of progress toward resolution.

  • Communicates in writing with Quality Manager to report potential quality care/patient safety issues identified in the course of data abstraction.

  • Documents and tracks identified problems, referring them to the Quality Manager for action and follow-up.

  • Accurately submits abstracted data to external sources, including but not limited to AHS Corporate Offices, Quality Net, Cerner, and CMS.

  • Downloads and reviews all external quality reports received from Federal and State governing bodies. Distributes reports to appropriate leadership and includes appropriate report summary.

  • Completes all review, data submission and reporting activities within the required time-frame.

  • Performs other related duties as assigned.

Qualifications

KNOWLEDGE AND SKILLS REQUIRED

  • Proficient in Word, Excell, PowerPoint and Cerner

  • Able to communicate effectively in ENglish, both verbally and in writing

  • Manual dexterity to operate keyboard

  • High degree of concentration to read and interpret medical records effectively and abstract and enter data accurately

EDUCATION AND EXPERIENCE REQUIRED :

  • Bachelor’s degree in nursing, respiratory therapy, or laboratory science

  • ­3 years of recent experience in an acute care setting.

EDUCATION AND EXPERIENCE PREFERRED:

  • One-year experience in data collection and analysis

  • Knowledge of federal and state quality measures

  • Master’s degree

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED :

  • Registered Nurse in the State of North Carolina

LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:

  • Certification in Healthcare Quality

JOB SUMMARY

The Quality Outcomes Analyst is responsible for implementation, management and maintenance of a comprehensive program of data collection and analysis of clinical care for internal and external benchmarking to improve patient outcomes and hospital processes. Identifies opportunities for process improvement and collaborates with leadership and direct care providers to advance quality initiatives. Participates in or facilitates PI teams as requested.
