You Will be Responsible For:
· Patient/support partner(s) and staff education: Promotes knowledge of surgical procedures and effects for patient, support partner(s), and care providers
o Meets with patient/support partner(s) prior to scheduled surgery to provide education on surgical procedure, post-operative expectations and care needs.
o Collaborates with direct care staff to provide ongoing education for surgical procedures, evidenced-based practice and/or research developments.
o Provides discharge teaching and arranges post-discharge care resources and supplies.
o Contacts patients/support partner(s) after discharge to follow up on any post-operative care needs.
o Collaborates with existing community organizations on educational initiatives.
· Quality Management: Educates and collaborates with multidisciplinary team to plan and participate in quality initiatives and research projects.
o Reviews patient information for pre-existing medical conditions and risk-factors.
o Serves as customer service advocate for patient and support partner(s).
o Consistently collaborates with physicians and multidisciplinary team to establish and oversee plan of care.
o Reviews physician and nurse documentation to ensure quality measures are achieved.
· Clinical Services: Serves as primary point of entry for providers and patients requiring navigation and coordination of care. Collaborates with patient to identify potential barriers to achieving post-surgical health goals.
o Reviews patient information to follow up on diagnostic tests and obtains results of previous tests as needed.
o Monitors patient’s progress through post-operative phase to ensure patient reaches post-operative goals.
o Collaborates with nursing care department(s) to confirm continuity of care during non-business hours.
· Community Outreach: Contributes to community programs and education pertaining to specific surgical services
o Maintains a working knowledge of the availability of community resources and financial services for providing clinical care and reducing barriers to care.
o Fosters supportive professional partnerships with community physicians.
o Participates in select community events to promote specific surgical programs at Park Ridge Health.
o Collaborates with community resources commonly used for post-operative care needs.
What You Will Need:
Masters degree in Nursing (or enrolled in a program)
Professional certification in specialty area by recognized national nursing organization within 1 year of hire required
Current AHA BLS certification required
Current RN licensure in the state of North Carolina required
ACLS and PALS preferred
At least 3 years of recent nursing experience in acute care nursing required
Recent inpatient acute care experience on a post-surgical unit preferred
The Surgical Nurse Navigator is a specialized health care professional who guides specific surgical patients through the healthcare system during and surrounding the time of surgery. The Navigator will focus on four areas of service: patient/support partner(s) and healthcare provider education, quality management, clinical services, and community outreach.
