Work Hours/Shift:

You Will Be Responsible For:

  • Communicate with physician regarding complex discharge planning or psychosocial issues which impact the plan of care.

  • Assist with the completion of discharge planning and psychosocial assessment within 48 business hours of admission when acting as the Case Manager with complex discharge planning.

  • Through supportive intervention and education, facilitate patient/family decision-making and actions needed to accomplish a safe and appropriate discharge plan .

  • Provide interventions for end of life decision-making and act as liaison with the Ethics committee.

  • Demonstrate ability to interpret complex psychosocial phenomenon.

Qualifications

What You Will Need:

  • Master’s degree in Social Work from an accredited School of Social Welfare.

  • One year hospital Social Work and experience coordinating patient discharge planning in a hospital.

  • Three years or greater Hospital Social Work and experience coordinating patient discharge planning in a hospital.

Job Summary:

The Masters Social Worker at the LTAC under Select Medical Case Management support will assist or act as the Case Manager in meeting the psychosocial needs of patients and families and facilitate discharge planning and coordination in situation where more complex post-hospital care needs exist. Provide counseling to address the impact of illness on the patient/family system. Apply knowledge of age/developmental issues to assist in the patient’s adjustment and healing. Review specific populations for psychosocial needs for the elderly where little or no support system exists, all at risk patients suspected for potential abuse, neglect or exploitation. Provide grief counseling with patients struggling with end of life issues, i.e., terminal disease state or fetal demise. Provide assistance to Case Managers to facilitate new Nursing home placement issues. The MSW will act as the Case Manager and coordinate the plan of care among all members of the health care team as required. The MSW Case Manager Conducts Interdisciplinary Team Meetings; Conducts C3 conferences; Works with the Case Management team, physicians and clinical care team to plan discharges for complex medical patients; Interacts with payors to receiving concurrent authorizations for the patient stay and plans discharges in conjunction with the payor; documents the patients clinical picture related to discharge planning; documents ongoing stays for commercial patients and utilization review for Medicare patients in the appropriate software systems; contributes to the weekly outlier review meeting; escalates complex discharge planning cases to the COO and corporate resources. The Case Manager must have the professional ability to practice under minimal supervision. The MSW Case Manager must meet Department expectations to affect practice variations and enhance patient outcomes
