The role you will contribute:

Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

• Assess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data.

• Develop and implement patient plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team.

• Manages resources and patient flow through matching clinical competencies with individualized patient’s and unit needs.

• Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care.

• Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues.

Qualifications

The expertise and experiences you’ll need to succeed :

Required:

• Education: Master of Social Work (MSW)

• Cardiopulmonary Resuscitation (CPR)

• Current Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED), preferred for facilities without an AED

• Current Driver’s license

Preferred:

• 6 months home care/hospice experience

• Two years post Master’s experience in health care or related field
