Work Hours/Shift:

You Will Be Responsible For:

  • Understand populations, quality program and contractual deliverables.

  • Work collaboratively to prioritize work to meet quality and contractual goals.

  • Perform analysis of population and program data to identify key areas of opportunity.

  • Keep abreast of the evidence-based guideline recommendations by various external subject matter expert entities.

  • Research and evaluate clinical literature relating to developed pathways, guidelines and protocols to identify best practices.

  • Able to identify opportunities and gaps for improvement.

  • Research and utilize external clinical data sources where available to assist in various quality management or disease management projects.

  • Prepare written analysis and recommendations for future pathway development to the Quality and Utilization committees.

  • Develop (and revise) evidence-based protocols, clinical pathways, and guidelines.

  • Design, develop and implement process improvement initiatives.

  • Make recommendations on updates and changes to health system supported ambulatory electronic medical records to include protocols, tools and documents recommended by the QCI committee.

  • Make recommendations on updates and changes to the disease registry to support both Quality and Process performance improvement initiatives.

  • Develop tools to enhance performance for the various quality initiatives.

  • Facilitate Change Management.

  • Perform Clinical Chart review/audits to identify opportunities and support initiatives.

Qualifications

What You Will Need:

  • Bachelor degree in a Healthcare, Allied Health, or clinical discipline such as Nursing, Advanced Practice RN, Physician Assistant or other

  • Minimal 3 years clinical experience with progressively responsible practice experience, and demonstrated results relevant to the job accountabilities

  • 2-5 years of Clinical and Operational performance improvement analysis

Job Summary:

The Quality Improvement Specialist is part of the PHSO multi-disciplinary team that is responsible for improving both the individual health and wellness of program participants and the overall health status of a client's population. As part of the PHSO Quality team the Quality Improvement Specialist role is focused on improving clinical performance across all Network Regions by analyzing data, by population in order to identify key areas to drive improvement and to reduce variation in how care is designed and provided. The Quality Improvement Specialist will review, analyze and track clinical, utilization and financial data from multiple sources in order to identify opportunities and determine the activities needed to meet quality targets and employer and payor contractual goals across the continuum. Working collaboratively with the Medical Director, Finance and Quality leaders, the QI specialist will research, identify best practices and develop protocols, procedures and guidelines as well as facilitate process improvement initiatives to drive performance across populations to achieve the goals of the assigned populations served to meet the organizations Quality program and contractual requirements.
