Full Time Day Shift
You Will Be Responsible For:
Coordinates care of patients in the program to ensure quality clinical outcomes achieving shorter length of stay, low morbidity and mortality rates as well as patient satisfaction and post-dc calls as applicable.
Coordinates and provides physician, staff, patient and community education as applicable; evaluates the effectiveness of these programs when possible.
Consistently uses current evidence-based guidelines, standing orders, and protocols to facilitate positive patient outcomes.
Collaborates with other in-house departments and community resources to provide continuity of care, as indicated.
Develops, monitors, analyzes and reports program performance improvement indicators and initiates process improvement as needed.Maintains ongoing QI plan.
Acts as an internal and external consultant on program and patient population related initiatives.
Coordinates the entire program to promote and improve the quality of care for the designated specialty population.
Communicates new evidence-based guidelines and regulatory standards to the program committee and assists in updating current policies, protocols, and order sets.
In conjunction with patient care, is involved in planning and content of patient/family education materials.
Effectively utilizes internal and external benchmarking for data collection and data analysis.Provides analytical interpretation of data and analysis of variances in a timely manner to appropriate customers.
What You Will Need:
Strong analytical and organizational skills to prepare and maintain various documents/reports.
Advanced level of clinical practice skills for area of specialty.
Teaching skills that reflect a knowledge of adult learning principles.
Excellent written and oral communication skills.
Proficient in the use of required clinical and program software and data collection.
Bachelor's degree required
Minimum of three (3) years clinical experience in area of specialty
Current Florida RN License
National nursing certification in specialty area required within two years of employment
AHA BLS- certified
The Bariatric Coordinator ensures that all necessary resources are in place to facilitate effective client support and treatments toward providing quality patient care. This means they interact daily with patients, physicians and nursing staff. The Bariatric Coordinator is responsible for all program management, including staff and community education if applicable, patient rounds, outcomes monitoring, preparation of statistical reports, data abstraction and maintenance of all required documentation; and compliance with national standards, certifications, and accreditations.
