Florida Hospital Ocala opened its doors in 1898 as Marion General Hospital and has been in continuous operation since then. Then in 1928, the hospital was renamed to Munroe Memorial Hospital. Adventist Health System acquired Munroe Regional Medical and renamed it Florida Hospital Ocala in 2018. GENERAL SUMMARY:

Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals.

What you will responsible for:

  • Hand washing when entering and leaving patient's room

  • Completion of utilization review on all patients per UM plan

  • Use of Discharge Planning Evaluation

  • Use of Curaspan on discharge planning

  • Consistent utilization of Patient Choice Form

  • Completion of High Risk Screening Tool (LACE) on all Inpatient admissions

  • Completion of 30-Day Readmission Tool

  • Completion of Important Message from Medicare Letters

  • Completion of Appeal of Process

  • Participates in Daily Safety Huddles

  • Obtaining completed Inpatient admission orders on all Medicare patients

  • Utilizing electronic and telephonic communications effectively: monitors emails and voicemails and responds in a timely manner.

Qualifications

What you will need:

  • Associate Degree in Nursing; BSN preferred.

  • One year of recent acute hospital case management experience required.

  • Current FL RN License required; ACM or CCM preferred.
