You Will Be Responsible For:
Attends the all training programs for required data collection.
Receives selected identified medical record list from corporate office based on sample size of records to be abstracted on a monthly basis at facility.
Familiar with all parts of the medical record and chart review.
Reviews and abstracts information timely.
Validates that diagnosis supports record to be included in abstraction process and appropriate patient population.
Ensures medical record is complete for abstraction process. Only use reports and dictation completed in the normal course of chart completion.
Communicates with physician offices, nursing homes, home health, and clinics as appropriate to obtain the additional information that may be needed to complete abstraction.
Forwards a list of all cases with identified outliers, following validation, to the Quality Director, Department Managers and others as appropriate.
Communicates concurrent quality measure variances to the appropriate staff for actions/resolutions.
Reports any unaddressed concurrent variances timely to the Quality Director, Department Managers and others as appropriate.
Responsible for maintaining a current abstraction manual.
Other duties to be performed as requested.
What You Will Need:
Independently able to navigate electronic health record and abstract defined data for various cardiac registries as assigned.
Able to analyze and interpret data and provide executive summary of opportunities and strengths.
LPN or RN
Previous acute care clinical experience required. Attention to detail and follow-up necessary. Must be emotionally mature and able to function effectively under stress. Strong interpersonal skills, and the ability to communicate effectively with patients, families, hospital staff, physicians and community resources is required. Must be able to work independently.
Current Florida LPN or RN license required.
Concurrent and retrospective abstraction of identified CV Data and charts. QA and report follow-up with physicians and team members to help ensure all elements are accounted for. Participates in CV team meetings to identify opportunities for PI within the service line.
