Work Hours/Shift:

Full Time

You Will Be Responsible For:

  • Hand washing when entering and leaving pt.'s room

  • Use of Discharge Planning Evaluation

  • Use of Curaspan on discharge planning

  • Consistent utilization of Patient Choice Form

  • Completion of High Risk Screening Tool (LACE)

  • Completion of 30-Day Readmission Tool

Qualifications

What You Will Need:

  • Master’s Degree in social work required

  • In lieu of an MSW the following will be accepted: Master’s Degree in Marriage and Family and/or Master’s Degree in Mental Health

  • One year of case management experience required for an MSW and 2 years in an acute care setting required for all other degrees

  • LCSW (preferred)

Job Summary:

Utilizes Social Work skills to assess patient and family functioning and emotional well-being in order to develop appropriate and timely discharge plans, community referrals, and provide psychosocial support. In collaboration with the physician, nursing, therapy staff, pharmacy and RN Case Manager, provides assessment planning and interventions to optimize quality of care, utilization costs and patient satisfaction.
