Job Summary:

The AdventHealth Neuroscience Institute's Brain Attack Center model is structured to achieve established objectives using a team approach supported by a system-wide Brain Attack Center Manager and Coordinators. The Brain Attack Center Coordinators, in collaboration with medical and clinical leaders, are responsible for the coordination of clinical excellence in stroke care across the AdventHealth 7-campus system. At multiple assigned campuses the Coordinator functions as a key decision maker for the interdisciplinary team, which is responsible for monitoring and improving program performance. The Coordinator facilitates the development and implementation of stroke-related educational initiatives for staff, patients, community and emergency medical services. The Coordinator guides interdisciplinary team members in performance improvement and the incorporation of evidence-based practice protocols into the daily operations of the stroke program at their assigned AdventHealth campuses. The Coordinator assists in ensuring a high level of data accuracy and quality for stroke program reporting. The Coordinator works with community and professional organizations to further community education and awareness about stroke prevention and treatment. The Coordinator actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Knowledge, Skills, Education, & Experience Required:

  • Advanced knowledge of adult neuroscience, stroke treatment, care and prevention

  • Strong communication, collaboration, and conflict resolution skills

  • Aptitude for critical thinking, decision making, and managing multiple competing priorities

  • Strong facilitation skills and a high level of self-direction

  • Ability to interact appropriately with patients, families, staff, physicians, and the community

  • Graduate of school of nursing or related clinical healthcare field

  • Three to five years experience in neuroscience nursing or related healthcare field involving neuroscience (e.g. rehab therapy, emergency or critical care nursing, research coordinator)

  • Primary Stroke Center quality measures (Preferred)

  • Process/Performance Improvement (Preferred)

  • Experience as Stroke Coordinator (Preferred)

  • Associate’s, Bachelor’s or Master’s degree in Nursing or clinical healthcare field (Preferred)

  • Data management experience (Preferred)

  • Teaching experience in formal/informal settings with knowledge of adult learning principles (Preferred)

Licensure, Certification, or Registration Required:

  • Current, valid licensure in either:

  • Florida State or multi state Board of Nursing as a registered professional nurse, or

  • Board registration in Physical, Occupational or Speech Therapy

  • Certification in Neuroscience Nursing (CNRN) or Critical Care Nursing (CCRN) (Preferred)

  • NIHSS certification (Preferred)

Job Responsibilities:

  • Continuously evaluate the status of the brain attack program and assist with establishing program goals and improvements.

  • Exercise independent judgment in identifying opportunities and striving for innovative strategies to improve stroke patient care and outcomes.

  • Collaborate with the interdisciplinary team to effect change and implement interdepartmental initiatives to improve clinical care processes, increase patient satisfaction, and ensure best outcomes.

  • Responsible for coordinating, educating, monitoring and evaluating the response to inpatient new strokes.

  • Maintain a system for coordinating and tracking the continuum of stroke care for patients within the AdventHealth system.

  • Conduct timely follow up at assigned Primary Stroke Center and stroke-ready campuses to identify and assess stroke/TIA patients in order to influence adherence to clinical standards.

  • Facilitate communication among departments and role model critical thinking by applying active and proactive problem solving to complex clinical and operational issues (i.e. deviation from protocol/processes, stroke patient issues, staff and patient educational needs, customer service issues, etc.)

  • Perform follow-up phone calls after discharge to assess effectiveness of program in helping patients understand and comply with lifestyle changes for future stroke prevention.

  • Collaborate with the interdisciplinary team to assess and correct anticipated deficiencies in preparation for and maintenance of Primary Stroke Center designation.

  • Function as liaison to the stroke medical director, administration, neurology, neurosurgery, and all members of the acute stroke team.

  • Serve as a primary resource and provide direction for interdisciplinary involvement in stroke patient care.

  • Distribute regular feedback to involved parties about patients who have received acute stroke treatments.

  • Coordinate training of interdisciplinary team members to complete patient record review for quality performance measures related to stroke.

  • Help identify staff and community education needs, as well as develop and provide stroke related education to ensure compliance with Primary Stroke Center annual requirements.

  • Provide situational education and mentoring to advance staff knowledge in the promotion of quality patient care.

  • Assist with education of patients and their families on post-stroke/TIA care and risk factor modification during rounds on assigned Primary Stroke Center campuses.

  • Utilize various methodologies to enhance stroke awareness throughout the community.

  • Serve as a liaison to EMS, community agencies, and other stroke care facilities in regards to current trends for patient care and transportation protocols.

  • Distribute regular feedback about patients who have received acute stroke interventions.

  • In collaboration with the interdisciplinary team monitor and update stroke practice order sets and clinical pathways utilizing American Stroke Association, Brain Attack Coalition, and Primary Stroke Center quality measure guidelines.

  • Assist in the development of educational plans for communicating updates of stroke related evidence based protocols and processes across all ADH campuses.

  • Maintain knowledge of current trends, practice changes, and research findings related to the stroke patient population and demonstrate application to clinical practice.

  • Monitor ongoing documentation of patient care to ensure compliance with regulatory and Primary Stroke Center quality measure requirement.

  • Analyze practices that contribute to variance, draw conclusions from aggregate data, and assist in developing action plans utilizing best practices and research findings.

  • Present stroke program reports and action plans at committee meetings as needed.

  • Serve as a content expert resource to the data management team for stroke-related performance improvement initiatives.

  • Maintain current knowledge of the regulatory and Primary Stroke Center quality measure reporting requirements.

  • Train and mentor staff in the abstraction and analysis of stroke clinical performance and process measures.

  • Assist with stroke related information requests submitted through the NSI website

  • Assist with hospital-associated stroke support groups

  • Address the concerns of stroke patients and their families during rounds on Primary Stroke Center campuses or through follow up phone calls to identify needs and provide resources

  • Serve as a patient advocate to promote patient access to services, tools, and medications needed to ensure successful stroke care management.
