AdventHealth Orlando seeks to hire a Community Care Manager who will embrace our mission to extend the healing ministry of Christ.

Facility Profile:

Located on a lush tropical campus, our flagship hospital, 1,107-bed AdventHealth Orlando, serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. We are already one of the busiest hospitals in the nation, providing service excellence to more than 32,000 inpatients and 125,000 outpatients each year.

Monday – Friday; 8:00am – 5:00pm

Job Summary:

The Manager of Community Care will be directly involved with the planning and implementation of the AdventHealth Community Care program (FHCC). The program scope is the entire Central Division-South Region. Directly manages the FHCC Staff. Responsible for managing and improving the clinical operations of FHCC. Develops and fosters relationships with local Universities to implement University Health Coach program. Promotes teamwork and collaborative relationships between facility Community Care teams and other facility representatives to support the effective, efficient delivery of services and care coordination efforts. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Knowledge, Skills, Education, & Experience Required:

  • Strong organization skills

  • Excellent analytical and problem-solving skills

  • Effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of staff & community partners

  • Effective computer skills, particularly Microsoft Office Outlook, Word, Excel

  • Ability to work in a matrix-management environment to achieve organizational goals

  • Professional knowledge of management theory and practice

  • Knowledge of and compliance with the Patient Bill of Rights, Corporate compliance, OSHA, AHCA, HIPPA and TJC requirements as they relate to this field

  • Disease management/adult chronic disease/ health management background preferred. (Preferred)

  • Bachelor’s degree in clinical discipline such as nursing, social work, mental health or related/relevant degree

  • 2 years supervisory or management experience

  • 2 years relevant clinical experience

  • 2 years of previous management experience in a community setting (Preferred)

  • 2 years of Community clinical experience preferred such as home health, hospice, mental health, or community social work (Preferred)

Licensure, Certification, or Registration Required:

  • Current, valid State of Florida or multi state license as a Registered Nurse, Licensed Mental Health Technician or Licensed Clinical Social Worker

Job Responsibilities:

  • Manages the Community Care program throughout the Central Division-South Region. Responsible for managing and improving the clinical operations of AdventHealth Community Care (FHCC) for the Central Division-South Region. Develops and fosters internal and external relationships with key stakeholders, including physicians, staff, leadership, community partners and universities.

  • Manages the facility Community Care Staff including hiring, scheduling, orientation and any positive disciplinary processes. Promotes teamwork and collaborative relationships between facility Community Care teams and other facility representatives to support the effective, efficient delivery of services and care coordination efforts.

  • Responsibility for Legal Compliance and Confidential Information: Confidential patient information is used routinely throughout the day. The Manager is responsible for the compliance with all state and regulatory governing bodies.

  • Use of Resources and Confidential Information: Ensures patient information confidentiality is maintained. Ensure protection from loss, theft or damage of supplies and equipment that may be used in the patient homes. Must assure that all purchasing decisions are made with regard to the highest ethical standards and corporate compliance.

  • Responsible for management of operating budget and ensuring program stays on budget. Provides Executive Director input on budgetary needs and potential capital budget items.

  • Implements strategic plan for FHCC with assistance of Executive Director. Provides input to Executive Director on strategic plan goals and implementation.

  • Monitor productivity metrics and coach staff for improvement as needed. Monitor outcomes metrics and coach staff for maximum effectiveness. Determine root cause of any difficulties with meeting performance objectives and work with staff or other stakeholders to alleviate or mitigate.

  • Acts as liaison with local Universities in regard to University Health Coach Program. Fosters relationships with local clinics and physicians that care for FHCC patients.

  • Directly supervises facility Community Care Staff and interdisciplinary teams. Indirectly supervises and provides guidance as needed for Student Health Coaches and other interns within the FHCC program. Monitors and coaches staff for improved performance and meeting program goals.
