You Will Be Responsible For:
SCOPE OF RESPONSIBILITY :
Provides leadership and strategic direction to the Advent Health Diabetes Institute (AHDI).
Collaborates with AdventHealth Diabetes and Endocrine Clinics, Translational Research Institute for Metabolism and Diabetes (TRI), CREATION Health, Alliance and Innovation Exchange, community partners and hospital departments to maximize opportunities for improving the health of our community and lowering the cost of healthcare through the management and prevention of Diabetes and Obesity.
The deployment of a whole person screening and resourcing to assess whole health-related social needs
Staffing responsibilities of team inclusive of diabetes education/schedulers, culinary medicine team, commercial bundle, and whole health team members
Establishes centralization of health and wellness messaging and programs with Advisory Board
Develops and Operates whole health programmatic interventions inclusive of functional medicine, diabetes education, and lifestyle medicine.
Measures outcomes through research and program evaluation partnerships.
Responsible for the partnerships for behavioral health services and ensuring services are meeting contractual standards.
Physician, Clinician, and Student education on Population Health, inclusive of whole health
Oversee operational function of AHDI Diabetes Education, Center for Nutritional Excellence, New Day New Weigh weight management and Thrive Diabetes Corporate Care Program
Provides strategic and operational direction for all aspects of AHDI including strategy, business development, education programs, core operations and divisional diabetes alignment strategies
COMPLIANCE/REGULATORY RESPONSIBILITY :
Ensures staff compliance with HIPPA
Ensures compliance with CMS Conditions of Participation as needed
Responsible for maintaining required American Diabetes Association accreditation recognition in good standing for seven Florida Hospital Campuses and regional and state affiliate programs
OPERATING & CAPITAL BUDGET/FINANCIAL RESPONSIBILITY :
Meets budget expectations for the Whole Health programs and services
Works with finance team for ROI (inclusive of downstream financials)
Ensures appropriate analysis and corrective action to meet financial metrics
Coordinates any necessary capital spending related to new space, new equipment, or spending of foundation accounts for capital needs for the AHDI
STRATEGIC PLANNING RESPONSIBILITY :
Responsible for development of long- and short-term business goals and strategies for growing relationships with providers and clients to ensure growth and success of Whole Health and AHDI
Leads client partnerships to further improve the health of our community and reduce the per-person cost of healthcare by deploying and developing programs that focus on whole health, diabetes management for large group, an innovative approach to diabetes education, diabetes prevention and obesity
Oversees the AHU & GME curriculum build and deployment for population health
PERFORMANCE IMPROVEMENT RESPONSIBILITY :
Performs ongoing review of all program operations and effectiveness and leads change efforts accordingly.
Provides oversight and direction to the leadership team for development and implementation of performance improvement activities.
Partners with Physicians and Provider Support for practice process improvement in relation to Complete Care Service Delivery Expectations.
Identifies opportunities for new revenue streams to support Whole Health/AHDI programs
COMMUNITY RELATIONS RESPONSIBILITY :
Develops of partnerships and serves as the executive liaison and consultant to AHDI related corporate and community partners.
STAFFING/LABOR/SUPERVISORY RESPONSIBILITY :
Ensures smooth operations within AHDI as the defined scope of: diabetes corporate care programs, weight management program, center for nutritional excellence, diabetes educational offerings, and additional endocrinology programmatic support
Oversees the leadership team responsible for daily operations in compliance with financial critical success factors.

KNOWLEDGE AND SKILLS REQUIRED :

Thorough knowledge of social determinants of health in communities specifically related to mental illness, emotional disturbances, and substance abuse.

EDUCATION AND EXPERIENCE REQUIRED :
Bachelor’s Degree in Public Health, Social Work, Psychology/Behavioral Health, Healthcare, or Business
Five (5) years of functional experience
Three (3) years of program/project management or leadership experience
EDUCATION AND EXPERIENCE PREFERRED :
Master’s Degree in Public Health, Social Work, Behavioral Health, or Business
LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED :
Current registration with Florida Board of Nursing as a registered professional nurse.
