Work Hours/Shift:

  • Full Time, After Hours

You Will Be Responsible For:

  • Assessing and updating patient needs

  • Education to patients and families on POC and changes

  • Visits for emergencies, routine care, level of care changes

  • Phone triage for mediation management, questions

  • Documentation accuracy per Medicare guidelines


What You Will Need:

  • Registered Nurse in the state of Florida

  • Must maintain active Florida driver's license and auto insurance

  • BLS

Job Summary:

Performs responsibilities of coordination and supervision of hospice services to patients after hours and when the office is closed. Performs intake function, scheduling and clinical support for clinicians working. The RN provides skilled professional nursing care and support to patients and families after hours as ordered by physician and in compliance with regulations and standards. The RN provides care when needed as issues and/or emergencies arise. The RN follows the plan of care and assesses for needed services or changes in level of care. The RN assumes responsibility for implementing POC updates with collaboration from the physician on call or attending on case. The RN is responsible for accurate documentation for all visits. The RN may provide visits upon expirations.
