Schedule: Part Time
Shift : 16-24 Hours per week. 8-5:30pm, days open.
Location: This is a fieldwork position covering Flagler and Volusia counties

The role you’ll contribute:
The Hospice Social Worker delivers various social work services to hospice patients and families. The purpose of social work intervention is to provide initial psychosocial evaluation, ongoing psychosocial support, direct casework services, and referral to community resources to assist with end-of-life care needs, as well as to engage in community education and outreach. Through collaboration with the hospice interdisciplinary team, the Hospice Social Worker implements, evaluates, and changes as needed individualized plans of care. He/she assists in identifying and implementing measures and interventions to assist patients and families in reaching desired outcomes. The Hospice Social Worker reports to the Patient Care Manager.
You Will Be Responsible For:
Demonstrates critical thinking skills, analysis, and sound clinical judgment in the care of hospice patients/families and uses the social work process to address the physical, emotional, psychosocial, and spiritual needs of patients and families.
Conducts a systematic assessment of the physical, psychosocial, and functional aspects of the patient and family and their impact on potential outcomes.
Determines patient/family problems, issues, and needs to provide an individualized plan of care with the collaboration of the interdisciplinary group.
Participates as a member of the Bereavement Team as assigned.
Maintains accurate, timely documentation and meets Hospice Medicare Conditions of Participation requirements.
Is proficient with electronic medical record documentation.
Assures compliance with payor source guidelines.
Acts in an educational and advocacy role, in patient care, with peers and in the community.
Recognizes and respects diversity and the influence of personal and cultural values on perceptions regarding health care and their impact on the delivery and quality of care.
Demonstrates flexibility, responsibility, and accountability to promote the well-*being of the hospice patient/family and the hospice team.
Qualifications
The expertise and experiences you’ll need to succeed:
Graduate of a Master's program in Social Work required.
Must have one year experience in a healthcare setting.
Must be licensed or eligible for licensure within the State of Florida (LCSW).
Must maintain active driver's license and auto insurance.
Hospice experience preferred.
