The role you ’ll contribute :

The RN Case Manager will utilize clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination, utilization management, and patient advocacy. Responsible for assuring medical appropriateness criteria are met for status and level of care.

• Reviews & analyzes informationrelativeto admissionin accordance withAdventHealthpolicy anddocuments assessment using case management software and/or other clinical informationsystem.

• Assesses patient’s physical, psychosocial, culturaland spiritual needs through observation, interview,review of records and interfacing with patient, physician and interdisciplinary team and caregivers toassistpatient/family in making decisions toward next level of care.

• Reviews & analyzes information relativetoutilizationmanagement when applicable.

• Facilitates discharge planning using case management software, working with patients, families andtreatment team making any needed referrals/ arrangements and documenting actions.

Qualifications

The expertise and e xperiences you’ll need to succeed:

• Associate DegreeNursing

• Nursing Diploma

• 1 year of nursing or case management experience required

Preferred Qualifications:

• BachelorDegreeNursing

• Experience with EMR

• Working knowledge of regulatory requirements and accreditation standards
