Work Hours Shift:
FT days, 40 Hours/week
M-F, 7:30am-4:00pm

You Will Be Responsible For:
Common to BH Inpatient, IOP and BHAC:
  • Manages crisis situations as needed, demonstrating ability to quickly appraise crisis situation and select appropriate intervention.  
  • Identify and intervene for suspected abuse, including sensitivity to age specific concerns. 
  • Document in accordance with hospital, JCAHO and CMS Standards.
  • Consult on clinical issues with physician, treatment team, director and other department personnel.
  • Functions as liaison between patient, hospital and community agencies.
  • Effectively provides clinical information and documentation to managed care organizations during precertification and/or concurrent reviews, advocating for the patient based on clinical presentation. 
  • Demonstrates ability to use electronic medical record to document all aspects of patient's treatment.
  • Actively participates in Clinical Coordinator meetings.
  • Maintains solid clinical skills including knowledge of DSM-IV-TR and DSM-5, community resources, process of payor source identification, and short-term intervention strategies.
  • Demonstrates excellent communication skills when collaborating with physicians regarding patient disposition.
  • Assists in orientation of new team members.
  • Demonstrates ability to implement AIDET effectively, incorporating the 5 Fundamentals of Patient Communication during all interactions.

Inpatient & Intensive Outpatient Programs Specific
  • Assesses presenting problem, formulate clinical impressions and complete Psychosocial History.   The Psychosocial History will incorporate population specific details.
  • Demonstrates ability to synthesize assessment information into an Integrated Summary.
  • Prioritizes clinical problems, formulate treatment goals and facilitate construction of treatment plan, updating based on continuous evaluation and assessment of progress.
  • Formulates discharge plan in collaboration with the multidisciplinary treatment team, the patient and the family. Assure outpatient appointments are scheduled within 7 days of inpatient discharge. 
  • Plan and conduct individual, group and family therapy with each patient and/or family.
  • Selects appropriate interventions, coordinate implementation with treatment team and evaluate effectiveness.  
  • Coordinates the continuum of care of patients between Behavioral Health Programs.

Behavioral Health Assessment Center Specific
  • Provides clinical assessments for psychiatric and/or substance use issues through scheduled assessments in the Assessment Center, Emergency Department assessments, and consultations to medical floors (as directed by physician order).
  • Recommends and provides appropriate referral(s) based on information obtained during assessment process.
  • Demonstrates competence in the area of risk assessment and detailed documentation supporting recommendation.
  • Provides initial contact for individuals seeking assistance at the Assessment Center and initiates preliminary paperwork, including documenting accurately and completely on the Community Call Form and arranges appropriate referrals based on needs.
  • Documents detailed, accurate clinical information on the electronic medical record within the Behavioral Health Intake form, the BHAC database and in MIDAS.  
  • Accurately enters charges on BH Intake form.
  • Assesses need for follow-up contacts with patient and initiates follow-up to pursue appropriate intervention.
  • Coordinates admission to a BH program in collaboration with psychiatrists, charge RN, and/or Intensive Outpatient Program (IOP) associates.
  • Maintains knowledge of protocols with agencies such as local Mental Health Centers (related to Medicaid evaluations) and the district Attorney's offices (related to involuntary commitments).
  • Provides for pager coverage and responds to pager within 10 minutes of call during scheduled shift or during "on-call" status.
  • Provides consultation on clinical issues with physician, treatment teams, department personnel and hospital-wide personnel.
  • Demonstrates knowledge of EMTALA guidelines and acts in accordance with standards.
  • Initiates call to Managed Care organizations, provides clinical review and obtains pre-certification information.
  • Maintains a professional interaction with Emergency Department personnel and inpatient floor personnel.
What You Will Need:
  • Experience working with a variety of psychiatric and addiction disorders, patient populations, and levels of acuity.
  •  Experience working with a continuum of care including inpatient and intensive outpatient programs.
  • Experience working with a multi-disciplinary team.
  • Experience with community resources.
  • Experience providing individual, group and family therapy.
  • Experience with assessment, treatment planning, documentation, and discharge planning.
  • Experience with crisis intervention.
  • Awareness of therapeutic boundaries and ability to monitor transference within the therapeutic relationship.
  • Patient-centered approach with ability to connect with each patient
  • Master's Degree
  • 1 to 2 Years of Position-Related Experience, 2-5 Years of Experience Preferred
  • Kansas Behavioral Sciences Regulatory Board License
  • License Addition Counselor OR Licensed Clinical Addition Counselor- Preferred
  • Crisis Prevention Institute (CPI) training (may obtain in first 90 days of employment

Job Summary:
The Behavioral Health (BH) Clinical Coordinator Team Lead functions in a role that leads the Clinical Coordinator Team in the development and implementation of structured and therapeutic program within the inpatient BH program.  The Team Lead role models professional and ethical behavior and strives to develop a highly-engaged team. Recognition, constructive feedback and crucial conversations occur between the Team Lead and the Clinical Coordinators.  Process improvement strategies will be designed and implemented with results shared with the Behavioral Leadership Team.  Customer experience initiatives will be designed by the Team Lead in conjunction with the Clinical Manager and the Clinical Director.  
In addition to the role of Team Lead, responsibilities will include providing coordination of care for a designated patient caseload.   This role may function in the BH Inpatient Program, the BH Intensive Outpatient Program (IOP) or in the Behavioral Health Assessment Center (BHAC).  Clinical responsibilities include provision of direct patient care through clinical assessments, individual therapy, group therapy, family therapy and/or discharge planning.  Additional responsibilities include enhancing the patient experience, program development, case management, utilization management, and participation in performance improvement activities. 

The Clinical Coordinator position will be assigned one of the following department responsibilities and may be assigned other duties/responsibilities as needed.
