Work Hours/Shift:

Part-Time

20hrs a week

You Will Be Responsible For:

Development, implementation, and daily operations of Cardiometabolic wellness program.

  • Develops and implements participant education materials (print, podcasts, video, digital)

  • Markets program to primary care providers, corporate interests and 3 rd party payers

  • Tracts individual and aggregate participant outcomes

  • Provides 1:1 wellness consulting to program & HeartAware participants to include:

  • Create action plan tailored to each individual participant;

  • Focus on emotional and social aspects of behavior modification;

  • Promote positive lifestyle changes;

  • Empower participants with practical skills and motivation;

  • Educates participants the value of wellness;

  • Guide participant to set realistic short and long-term goals;

  • Is an accountability partner and mentor to participants while helping them track progress;

  • Provide support to facilitate participant program success and goal achievement;

  • Address ideals such as habits, communication, coping, rewards, tracking, barriers and obstacles, support networks, confidence;

  • Development of communication pieces and participation reporting; and

  • Administrative duties on and off-site: emails and phone communication to program participants, coaching follow-up, miscellaneous administrative duties. Fulfill any other duties reasonably requested by SMMC. Build a relationship with participant to inspire positive change.

  • One-to-One Counseling can occur in different methods.

  1. In person counseling

  2. Telephone or Skype counseling

  • Assumes responsibility for daily operations of the LIVE WHOLE cardiometabolic program and Heart Aware coordination

  • Provides supervision and direction to participating staff.

Qualifications

What You Will Need:

  • Kansas Registered Nurse License

  • Bachelor’s Degree in related field

  • 2 to 5 Years of Position-Related Experience

  • Obtains Wellness certification within 6 months of hire

  • Basic EKG knowledge

  • Basic Exercise physiology knowledge

  • Basic cardiopulmonary knowledge

  • Ability to function independently with little or no supervision

  • Ability to communicate effectively in both written and oral form

Job Summary:

The Cardiometabolic LIVE WHOLE wellness coordinator has accountability, authority and responsibility for quality of services, operational management, and human resource management for the Heart Aware and the LIVE WHOLE Cardiometabolic Wellness Program. The coordinator fosters the integration of the organization’s vision and values, and accomplishes the mission and strategic plan through a collaborative and accountable work team. As the coordinator of the cardiometabolic program and community cardiovascular prevention initiatives the Coordinator collaborates with other departments, patients/customers, families, physicians, administration and associates to meet customer needs and expectations in a WHOLE person focused and healing environment.

In addition to the LIVE WHOLE Cardiometabolic program, the coordinator facilitates coordination with other preventative programs such as HeartAware and community education.

The coordinator is responsible the for the development and implementation of the LIVE WHOLE cardiometabolic program development, budget, planning, & staff competency in collaboration with other dept specialties and the executive director of CV services.
