Work Hours/Shift:

You Will Be Responsible For:

  • Assuring care gaps are met (for both healthy, and chronically ill patients)

  • Care measures are monitored according to standards set (i.e. diabetic patients obtaining hgbA1c’s, annual eye exams, and annual foot exams) and documented and reported appropriately

  • Patient relationships are developed, and are an integral part of care coordinators responsibilities

  • Provide follow-up contact(s) with patient(s) as indicated to assure compliance with recommendations (assuring medication reconciliations are done appropriately). This should include (but is not limited to) lab values, x-rays, specialist visits, PCP visits, dieticians, etc

  • Be able to run reports to determine patients that have not been in for appointments for up to two years, and activate process(es) to get them scheduled for an appointment

  • Monitor Medicare patients for their Annual Wellness Visits and establish a documented process to assure that patients are contacted about scheduling annual appointments

  • Ability to assess patient population needs through pre and post visit planning. This should include the coordination of any community resource needs, including language, transportation, medication access, etc.

  • Evaluate reports to determine patients in need of preventive care in a timely manner, and effectively document, monitor and report on these efforts, as directed

  • Establish an effective transition of care proves to assure that all patients discharged from the hospital are being seen in appropriate follow-up with their PCP (within 7 days)

  • Develop workflow and protocols with PCPs that assure ER and hospital follow-up is completed

  • Engage in the regular huddle and communication tools with practices that meet patient care needs (i.e. patient’s in need of mammograms, vaccines, flu shots, etc)

  • Engage all new patients in Care Coordination process, assuring that routine visits, vaccines, and patient engagement is achieved


What You Will Need:

  • Associate’s Degree in Nursing

  • 2 to 5 Years of Position-Related Experience

  • Kansas Registered Nurse License

Job Summary:

The Care Coordination Manager is responsible for optimizing the performance of the care coordination team and the chronic care management team. This will involve integrating clinical quality data from present IT systems with payer requirements, collaborating with physicians, office staff, and management on reporting data and performing analytics to improve overall patient care quality. Works to assist in report functionality to support care coordinators, and managed care functions. The Care Coordination Manager will also be responsible for leading and managing any quality initiatives through payors (i.e. CPC+ through CMS) or provider partnerships (any Clinically Integrated Networks).
